  5. Commodore Computer Club (UK)
  6. membership application.

  8. The on-screen version is in 40 colums,
  9. but readable.

  11. The Printout is 80 columns. Complete
  12. the form then mail it & your check to
  13. the address provided.

  15. The membership form also includes dues
  16. converted from \ to $ as follows:

  18.         6 months @ \3 = $5.70
  19.         1 year @   \5 = $9.50
  20.         Lifetime @ \30 =$57.02

  22. NOTE The program will not return to the
  23. Main Menu. When READY appears, type RUN
  24. to RUN it again. Otherwise, RESET your
  25. Computer & reload the disk.
